OAKLAND UNIFIED SCHOOL DISTRICT
To register as a volunteer with Faith Network, print and complete the volunteer registration form.

Then you will need to complete two screening procedures: 1) Negative TB test  2) LiveScan fingerprinting. Once you complete the screening process, mail a copy of your TB test results and LiveScan request form* receipt along with the volunteer registration form to Faith Network at the address below.

TB TEST
Volunteers who work directly with children must have a TB test and Livescan fingerprint. You can get your TB at your doctor’s office or medical clinic. Or you can make an appointment with Phyllis Fortuin, Clinic Director for City Team Ministries. She will provide TB testing on Tues. or Thurs., 10 am - 2 pm. A $5.00 donation recommended. Call for an appointment (510) 295-7405 (Phyllis’ cell) or (510) 452-3758 ext. 15. City Team is located at 722 Washington St., Oakland 94607.

LIVESCAN FINGERPRINT PROCEDURE
LiveScan fingerprinting is now provided by Prints on the Run, Alameda County Office of Education, and UC Berkeley Police Deptartment.  All fingerprinting fees can be paid for or reimbursed by Faith Network.  Faith Network will receive the DOJ response regarding the fingerprint clearance. Please bring the following to receive fingerprinting services:
•Live Scan Service Request form from Faith Network (call our office to request a form)
•Valid federal, state or local government picture ID (Driver’s license, passport, etc.)

Fingerprinting locations:

Prints on the Run is located at 580 Grand Ave. Suite S in Oakland.  Please call their office to schedule an appointment at (510) 268-9940.  Their hours are Monday - Friday, 9am - 6pm; and Saturday, 10am - 4pm.  Tell them to charge Faith Network’s account.

The Alameda County Office of Education Fingerprinting Office, located at 313 West Winton Avenue, Hayward, Room 172, offers Walk-In and Scheduled appointment. Phone: (510) 670-7711.

Scheduled Appointments:
Mon. & Fri. 7:35 –4:00 p.m., Tues., Wed. and Thurs. 1:00 –4:00 p.m.
Walk-In Availability:
Tues.. Wed. and Thurs. 7:35 - 11:00 a.m.(Except the 4th Wed. of each month)
Please call (510) 670-7711 to schedule an appointment. Walk-in is on a “first come, first served” basis for the first 30 applicants only. When you arrive, please sign your name in an available time slot.
No payment is necessary, Faith Network will be billed.

UC Berkeley Police Department Located at 1 Sproul Hall UC Berkeley Campus, Berkeley 94720
Faith Network volunteers/Cal Berkeley students can get fingerprinted thru UC police, located in the basement of Sproul Hall (Bancroft & Telegraph). Come to the font counter and someone will help you. Fingerprint apointments are made Tues. - Fri., 8:15 am - 4:15 pm by calling (510) 642-6760.  Bring a check made payable to U.C. Regents for $26.  Faith Network will reimburse you.


ALAMEDA UNIFIED SCHOOL DISTRICT
To register as a volunteer with Faith Network, print and complete the volunteer registration form.

All volunteers who provide services on a regular basis must be processed through the Human Resources Office prior to beginning volunteer service.  Volunteers must:

  • Be fingerprinted
  • Provide a T.B. test clearance (valid for four years).  T.B. kept at the site level.

LIVESCAN FINGERPRINT PROCEDURE

Volunteers are to be sent to the Human Resources Department to pick up the Fingerprinting Information Sheet and Livescan Form.   Fingerprinting is done through the Alameda Police Department (APD).  It is necessary to make an appointment to be fingerprinted by calling the APD (after 12:00 noon).  The APD phone number is 337-8433 and is located at 1555 Oak Street.

The volunteer should have a picture I.D. (driver license) on their person at time of appointment.

As fingerprinting is done in the jail area of the APD, volunteers should not bring their children and if there is jail activity in process at time of the volunteer’s appointment –their appointment will be cancelled and rescheduled.

Volunteers must pick up the Fingerprinting information Sheet and Livescan Form at the Alameda Unified District Human Resources Dept., 2200 Central Ave. 94501 (510) 337-7060. There is no charge for the Livescan.

The volunteer needs to bring back a copy of the livescan form to the Human Resources Department after their appointment.  This form includes pertinent information for tracking, should the Human Resources not receive fingerprinting information back within 7 days.

The Human Resources Department will notify the School Office Manager by e-mail when fingerprint clearance has been received.   Volunteers are not to start until clearance has been received from the HR Department.


BERKELEY SCHOOLS
To register as a volunteer with Faith Network, print and complete the volunteer registration form.

BERKELEY SCHOOLS VOLUNTEER ORIENTATION REQUIREMENT

Also, to work in the Berkeley public schools, volunteers must register with Berkeley School Volunteers, which requires a 1-hour orientation. The BSV orientations are conducted at 1835 Allston Way. You do not need to pre-register.  Visit their website for their orientation schedule at www.bpef-online.orgClick on “Volunteer!” to get to the schedule.

 


COMMUNITY ALLIANCE CHURCH

To register as a volunteer with Faith Network, print and complete the volunteer registration form.

Community Alliance Church volunteers must register with Faith Network and West Contra Costa Unified School District (WCCUSD).  Below is a checklist of procedures.

What you need to complete:

  1. Complete both pages of the WCCUSD Volunteer Application, sign and date
  2. Make a copy of entire volunteer application to send to Manny Martinez later (you may cover or leave blank your SSN# for this copy)
  3. Get TB clearance from doctor or clinic (no older than a year, keep current every 4 years)
  4. Contact Manny Martinez for a letter of approval from the school where you will be volunteering

Once you have completed the above you will make an appointment with WCCUSD Human Resources for LiveScan fingerprints.  To make a LiveScan appointment call:

Once you have made an appointment you will bring with you the volunteer application, TB clearance and letter of approval along with:

  • $32 in exact cash, personal check, or money order made payble to West Contra Costa Unified School District (reimbursed by Community Alliance Church)
  • Picture I.D. (i.e. drivers license, state I.D., passport)

Once you have been fingerprinted mail or fax the following to Manny Martinez:

  • Copy of completed volunteer registration form
  • Copy of completed LiveScan fingerprint form
